Background: Basal cell carcinoma (BCC) is a slow-growing, locally destructive malignant skin tumor. The nasal location considered high-risk feature BCC because its anatomical features problems identifying tumor preoperatively. Surgery cornerstone treatment various techniques developed combine full 3D removal with good aesthetic functional results. Case Presentation: We report circular black discoloration 64-year-old male patient on left side nose within 2 years. was scheduled surgical reconstruction defect using bilobed flap. flap consists two lobes common semicircular base marked around transverse portion nasalis muscle aponeurosis. then removes from triangular peninsula at defect. After removal, moved into placed 5-0 Prolene simple suture. Conclusion: has multipurpose all ways. main benefit this include similarity color texture relative site, ease reproducibility, low complication rates, ability design it different sizes tissue compositions.
